Can't tell if you're being sarcastic or not. The guys have stated multiple times that the original idea was conceived by Hetfield, the idea of a soldier having no arms, legs, etc, only his mind. Their road manager, or someone working on their tour team, mentioned Johnny got his gun to them -- thus why they use it in the video as well as base the idea around the book.

it's actually a song about a veteran from viet nam who had a land mine blow up a large percentage of his body and he actually survived the blast. and after that he was in a hospital and lost his sight and speach and hearing and his legs and one arm, and while in the hospital he would pound his head on the wall in morse code having known it through his military training the words, "kill me please" and the doctors thought that he was just going insane. and metallica wrote this song about it.
