What he said. I think what's more interesting is that the helmet fit her at all, considering it's equipment from a 'Con ship, and the Decepticons are all huge compared to the Autobots.

When she turns into Blackarachnia, she's got her back to the audience, so you can't really tell if she was already wearing the helmet or not. I think it's foreshadowing, not a mistake...
